Peer Evaluation

This is a confidential evaluation of all members of your team. It will be used to provide feedback to the team and its members. Please be consistent, honest, and as specific as possible in your comments. List team members (including yourself) in alphabetical order by first name from left to right. Enter a numeric rating from 1 to 5 in each box, where 1 = never, 3 = sometimes, and 5 = always. Remember to enter ratings for yourself.

For each team member, write a paragraph or two describing specific Strengths, items for Improvement, and Insights you’ve had regarding teams and projects. In particular, ratings of 1 or 5 should be supported by specific feedback.
